Immunizations
Due to the current health crisis, immunizations will be administered by appointment only. Call 815-544-9730 to schedule. We accept Medicaid. If child is uninsured or underinsured, please call in order to determine elgibility. We DO NOT bill private insurance except for the seasonal flu shot. If this is your first time here, print the immunization forms and fill them out completely in order to minimize the time inside the building. Please bring child's current immunization record.
The following vaccines are available for children:
- MMR - Measles, Mumps and Rubella
- Tetanus, Diphtheria, Pertussis (whooping cough)
- Polio
- Hepatitis B
- Haemophilus Influenza B (HiB)
- Pneumococcal
- Chicken Pox (Varicella)
- Meningococcal
- Rotavirus
- HPV - Human Papillomavirus
- Flu Shot (seasonal)
We accept Medicaid. If child is uninsured or underinsured, the cost per vaccine is $20.00. Services will NOT be denied for inability to pay. We bill Public Aid. We DO NOT bill private insurance. Please bring child's current immunization record.

Flu Clinics are held in the fall each year and clinic schedules are available after September 1st. We bill Medicare and Public Aid. Call the health department for costs and current information.
Following are immunization schedules as recommended by the Centers for Disease Control (CDC):
If you as the parent cannot come with your child to the clinic when they are to receive immunizations, and choose to send another adult with them instead, please fill out and send with them the below consent form that authorizes a non-parent to access routine health care treatment and services for your child in your absence. Parent must print and sign this form to send with the adult that will be bringing the child.
