Please apply for an Assumed Name/DBA through our website portal - https://ilboone.fidlar.com/ILBooneVS/Apex.WebPortal/
After completing the application online, please visit the Boone County Clerk & Recorder's Office between normal business hours of 8:30am and 5:00pm, Monday through Friday. Call with any questions at 815-544-3103.
All owners of the business must sign the DBA application. A valid Driver's License or State ID is needed for identification purposes. These individuals will be required to sign in front of a Clerk within our office.
The filing fee is $22 to be paid in our office. Payment forms accepted are cash and check only.
After receiving your Assumed Name/DBA Certificate of Intention - take the forms to the Belvidere Daily Republican or the Boone County Journal to be published within 15 days, one time per week for three consecutive weeks in the legal notice section. (Failure to publish within 15 days will result in a reapplication process.)
The fees for publication are the business owner's responsibility.

A Certificate of Ownership will be mailed from our office approximately one week after last published date is completed. The process takes approximately one month after the original form is completed within our office. The Certificate of Ownership shows that your business is registered in Boone County.
NOTE: Please confirm with the newspaper that all has appeared correctly stated in legal notice and the Certificate of Publication has been sent to our office in a timely manner.
*If any changes occur in ownership, address, or dissolution - these changes need to be updated and filed in the County Clerk's Office. Appropriate fees will apply.
