IMPORTANT: A Qualifying Life Event or a change in family status (such as a birth or marriage) is an event that allows County employees to make changes to their insurance elections during the plan year. Changes must be made within 31 days of the event. If you have an upcoming Qualifying Life Event or a change in family status, please contact Human Resources as soon as possible at 815-547-6269.

2023 Medical, Dental, and Vision Open Enrollment
Open enrollment begins on Wednesday, May 17th and employees have until Wednesday, May 31st at 12:00pm to submit the required forms to the Administration Office. If you are currently enrolled and do not wish to make any changes to your existing coverage, no action is required on your part.

2022 Tax Documents
IRS Form W-2 Wage and Tax Statement: Employees are able to access their IRS Form W-2's through their Paylocity account. Hard copies were distributed to Departments in January.
IRS Form 1095-C Employer Provided Health Insurance Offer and Coverage: Employees are able to access their IRS Form 1095-C's through their Paylocity account. Hard copies are available in Human Resources. To obtain a hard copy, please contact Human Resources.
